Qualified Housing Partner Timeline

If you operate a sober living or transitional housing program that aligns with Arizona’s standards for recovery housing, you may be eligible to become a Qualified Housing Partner (QHP). Approved partners play a vital role in helping our clients maintain stability and accountability as they continue their recovery journey. QHP’s provide housing specifically for justice involved, re entry individuals that are at risk of being homeless. Our Housing Voucher Administrator (HVA) will help you every step of the way.

Step 1 – Provider Interest to Become a QHP

  • If interested, send us an email at [email protected]
  • In your email, tell us about you and your program. Be sure to include what types of housing you have, how long you have operated in the Community and what cities and counties you operate in.
  • After you have sent your email, you can access the link in the autogenerated reply email or in the link or QR code below.

Step 2 – Qualified Housing Provider (QHP) Application

  • Access the QHP application form in the link or the QR code.
  • Your application is reviewed by Housing Voucher Administrator (HVA) within 2 business days of receipt.
  • HVA sends a determination email to interested provider and summary of next steps to include:
    • Property Inspection Process and review of the HUD Property Inspection Checklist
    • Memorandum of Understanding (MOU) Process
    • Invoicing & Outcomes
    • Onboarding and Informational Sessions
  • QHP Application or via the QR code:

Step 3 – Property Inspection Process

  • The potential QHP confirms the preferred date for inspection and acknowledges receipt of the HUD Property Inspection Checklist.

  • SRC identifies housing team member to inspect the property.
  • SRC completes onsite inspection using the HUD Property Inspection Checklist.
  • SRC approves or provides feedback for reinspection and emails results of Inspection and and/or reinspection process if necessary.

Step 4 – Memoradum of Understanding (MOU)

  • Once the housing inspection is complete and the property is approved, the MOU process is initiated and outlines the terms, roles, and responsibilities of each party and includes:
    • Outcome Measurements
    • Resident Survey Requirements
    • Deadlines
    • Invoicing
    • Meeting Requirements
  • Electronically signed by both parties through Adobe. The signer of the QHP must be an officer of the company.

Step 5 – Onboarding of QHP

  • Congratulations! You’ve made it this far and now prepare to accept your first resident.

Step 6 – Ongoing Coalition Meetings

A vital part of being a QHP, is attending our Coalition Meetings, where we have the opportunity to connect and collaborate and continuously improve every step of the way.

  • Monthly QHP meetings: Last Tuesday of the Month at 12:30 pm (virtually)
  • Outcome Data Reports to ADCRR
  • Discuss Successes and Barriers

If you are a COIII (Correctional Officer III), Parole or Probation Officer, attorney, or any other authorized referring provider and you know someone who may benefit from our treatment, treatment with housing, or housing-only programs, we welcome your referral.

SRC in partnership with ADCRR (Arizona Department of Corrections, Rehabilitation and Reentry), has launched a Housing Voucher Program (HVP) that provides safe, supportive housing to justice involved individuals. If you have an ADCRR number and are of risk of being homeless, we are here to help. Our HVP offers housing plus wrap around services to promote your physical and mental health, accountability, and community connections.
