Referral Timeline

If you are a COIII (Correctional Officer III), Parole or Probation Officer, attorney, or any other authorized referring provider and you know someone who may benefit from our treatment, treatment with housing, or housing-only programs, we welcome your referral. To ensure the referral process is smooth and timely, please follow the steps below:

Step 1: Submit a Referral

  • Identify the Appropriate Program
    • Treatment Program – For individuals who are in need of detoxification, residential or outpatient services, counseling, and recovery support.
    • Treatment + Housing Program – For individuals who require both treatment and safe and supportive housing.
    • Housing-Only Program – For individuals who are stable in their recovery to reside in a shared housing environment who only require wraparound services such as case management and peer support services.
  • Gather Required Information
    • Client’s full legal name, date of birth, and contact information.
    • Relevant legal documentation (e.g., convictions, probation terms, court orders, release conditions).
    • Summary of current status, needs, and any special considerations (medical, psychiatric, or behavioral).
    • Referral source contact details for follow-up and collaboration.
  • Click the link below to complete the Referral Form or email us at [email protected].
  • If you are already working with someone at Scottsdale Recovery Center, let us know on the application to prevent duplicate outreach.

Step 2: Wait for Our Response

  • We will contact you within 2 business days to discuss next steps.
  • If your referral is for treatment, our staff will contact you directly to schedule an intake assessment through our prescreen process.
  • If your referral is for housing only, our Housing Voucher Administrator will work with you to address housing availability, program fit, and any special circumstances.
  • If housing is not needed within 90 days, expect to hear from us within 5 business days so we can collect more details and schedule a follow-up closer to the move-in date.

Step 3: Resident Application for Housing Assistance

Step 4: Need Help?

  • If you have any questions regarding housing assistance, email us at [email protected].
  • We’ll send you an automated reply with helpful links and if you have already filled out the applications, please be patient and we will return your email within 2 business days.
  • If you have any questions regarding treatment, email us at [email protected] or call our reentry line at 1-888-409-0943.
  • Our goal is to provide a safe, supportive, and compliance-focused environment for every individual referred to us, ensuring they have the best opportunity to succeed in recovery while meeting their legal or correctional obligations.
