If you know someone who could benefit from our treatment, treatment & housing, or housing-only programs—and you are a probation officer, COIII, attorney, or other referring provider—please follow these steps:
Step 1: Submit a Referral
- Click the link below to complete the Referral Form or email us at [email protected].
- If you are already working with someone at Scottsdale Recovery Center, let us know on the application to prevent duplicate outreach.
Step 2: Wait for Our Response
- We will contact you within 48 business hours to discuss next steps.
- If housing is not needed within 90 days, expect to hear from us within 5 business days so we can collect more details and schedule a follow-up closer to the move-in date.
- We may be able to do a screening 90 days before if there are urgent needs.
Step 3: Resident Application for Housing Assistance
- If the person you are referring needs Housing Only – the individual you are referring needs to complete a Resident Application for Housing Assistance.

Step 4: Placement Decision
- Once we receive both applications, our team will review the information and determine the best placement for the prospective resident.
